are proud of our office, and we strive to provide you with the best dental treatment possible.
Please take a moment to read our office policy regarding payment
options and insurance billing procedures.
We accept Cash, Check, MasterCard, Visa, Discover, American Express
and Care Credit. We can assist you to apply for financing with Care Credit. Please
ask our staff for information on this dental finance program. The application process only takes a few minutes. Payment in
full is expected at the time dental services are rendered. We are preferred providers for Blue Cross/Blue Shield of Montana.
If you have Blue Cross/Blue Shield of Montana, Allegiance,
or Delta Dental, our office will do the billing. However, you will be asked to pay your estimated co-pay at the time of your
visit. Please provide the receptionist with your BCBS identification card.
For insurance, including Blue Cross/Blue Shield from other states,
we request payment in full at the time of service. Additionally, we do accept assignment of benefits from insurance
companies other than BCBS of MT. If you have given us your insurance information, we will provide you with a completed
insurance form for you to submit to your company.
Payments may not be delayed or withheld because of any insurance
coverage or pending claim status thereon. If there is a balance remaining after insurance pays, the balance is due and
payment in full is expected.
